 Includes: Assessment, written action plan, three scheduled family navigation sessions, service and treatment navigation, family boundary guidance, plan review and reasonable between-session communication.
60- Day Family Navigation- $2,750
For families navigating more complex situations that require continued guidance, strategy and support
Includes: assessment, written action plan, six scheduled family navigation sessions, service and treatment navigation, ongoing family boundary and strategy guidance, plan reviews, reassessment as circumstances change, and reasonable between-session communication.
90- Day Family Navigation- $3,750
For families navigating significant or evolving circumstances that require longer-term guidance, continuity and strategic support.
Includes: assessment, comprehensive written action plan, nine scheduled family navigation sessions, service and treatment navigation, ongoing family boundary and strategy guidance, coordination through treatment decisions and transitions, regular plan reviews, reassessment as circumstances change, and reasonable between-session communication.
